* Second year Initiation players are permitted to play in the Novice Division at the discretion of the team coach and parents.

Team registration deadline is January 31st, 2019

Final roster deadline is February 12th, 2019. Final rosters must include a minimum of 7 skaters and 1 goalie and may include up to 10 skaters and 2 goalies.

TEAM REGISTRATION FEES

A $1,000 non-refundable deposit is required at time of registration. The balance of fees will be due by January 31st, 2019.

*Registration may close before January 31st if the maximum number of teams in a Division has been reached.

COACH/ASSISTANTS

Please list the coach and any assistant coach(es)/helper(s) below. Note all coaches and assistants must provide proof of a valid and current criminal records clearance.

Note: Teams must provide at least one adult (19+) coach or assistant for all games and on-ice sessions.

By submission of this registration form, our team and all registered players, parents and coaches agree to the following:

The Youth 3-on-3 league hosted by OIC Enterprises Ltd dba The Ultimate Hockey Centre (hereinafter referred to as UHCis a competitive league that may not be suitable for some players.

Participation could cause my child or any children registered or others around them, harm, including, but not limited to, bodily injury, damage to property, emotional distress, or death.

Assume all risk of personal injury which may result from participation in the UHC Youth 3-On-3 League. Be advised that all UHC on-ice play requires full equipment. All participants and helpers (coaches, assistants, etc.) are required to wear a CSA approved helmet at all times while on the ice. Children (under19) are required to wear a CSA helmet at all times while on the bench.

Release, indemnify, and hold harmless the UHC, as well as all its employees, officials, agents, representatives, successors, etc. from all losses, claims, theft, demands, liabilities, causes of action, or expenses, known or unknown, arising out of participation in or attendance at the UHC.

Ensure all participants carry up-to-date private or government health insurance.

To grant and release the UHC the right to use photographs, audio tapes, and/or videotapes in which any registered players, coaches or guests appear in any materials such as videos, films, recordings, still photographs or articles relating to the UHC, its leagues and services including, but not limited to, brochures, newsletters, annual reports or any other advertising and/or promotional materials whether broadcast on television, radio or any other medium.

The UHC reserves the right to make changes to the 3-on-3 league program at any time, at its sole discretion. 

GENERAL SAFETY & LIABILITY POLICIES

The UHC is a smoke-free facility. Smoking is not permitted anywhere on the grounds

Alcohol, alcohol consumption & drugs are strictly prohibited at all times on UHC property. Users will be held responsible for any liability resulting from the consumption of alcohol by any persons connected or associated with their group.

When the ice surface is to be conditioned by the UHC staff, all players/coaches/referees will immediately leave the ice surface and remain off the ice until the ice machine has left the ice surface and an attendant has closed and secured the Zamboni gate. Water bottles and other equipment must not be left on the boards while the Zamboni is conditioning the ice. Minors are required to remain in the dressing room while the Zamboni is in operation.

Dressing rooms are to be tidied at the end of each session. All garbage and other materials brought on site by the user are expected to be removed from the building or disposed of appropriately by the user in a garbage can or recycling container. A fine may be levied upon the user if in violation of this policy. Equipment and other items left behind by users are subject to disposal by UHC staff.

Children under the age of 12 must be supervised by an adult at all times while using the facility. Children may not be dropped off and left at the facility without an adult from the child/children’s user group present.

All users agree to maintain discipline among their group members and ensure appropriate conduct while using the facility. Vandalism, abuse of staff and volunteers, littering and any other behaviour that is disruptive to other users and employees, or is in any way inappropriate or in violation of the policies outlined here will not be tolerated and may result in expulsion from the premises and/or future suspension of rental privileges and a fine.

Users will also be held liable for any damage to the facility or its equipment resulting from negligent use, irresponsible behaviour or violations of any of the policies outlined here by the user(s) or any individuals associated with his/her party. This policy extends to the family members, siblings and friends/associates of the user(s) attending the facility during the user’(s) allotted rental period.

The UHC is not responsible for any lost, stolen or damaged articles resulting from use of the facility.

Users must vacate dressing rooms no later than thirty (30) minutes following the end of the ice rental.

The UHC reserves the right to expel and/or refuse service, without reimbursement, to any individual or user group it deems, in its sole discretion, to be acting in violation of these policies or behaving in a manner that is disrespectful, abusive and/or poses a danger to staff, other players, officials and/or other users/customers/guests and/or poses a risk of damage to the facility and/or its equipment. We expect all players and guests using the UHC to treat the facility with the same respect and consideration they would expect observed at their own homes.
