If you are registering an individual player but would like to request they be placed on a team with another registered friend, please indicate the requested friend(s) below (you may request up to two players)

Teams must consist of a minimum 7 skaters and 1 goalie and a maximum of 9 skaters and 1 goalie.

ALL GAMES ARE COMPETITIVE BUT NON-CHECKING.

Fees & Cancellation

SINGLE INDIVIDUAL PLAYER REGISTRATION

Fee for Individual Player registration is due at registration. Individual player registration fee is non-refundable. Individually registered players cancelling before February 1st will receive a credit towards a future 3-on-3 Youth League registration. No credit will be applied for cancellations after February 15th. When possible, individually registered players will be joined with other individually registered players to form UHC created teams. Alternately, individually registered players may also be placed on already registered teams if those teams require additional players to fill out their rosters.

TEAM REGISTRATION

For team registrations, a $1,100 non-refundable deposit is required per team due upon registration. The balance of team fees will be due by February 15th, 2018. Registrants may continue to add players to their team roster until the registration deadline. To add a player to a team you've already registered, have the new player complete the registration form as an individual player registration and indicate in the comments section at the bottom that they are registering for an existing team and include the team registrant's name for reference. Note that if you fill your roster with 10 players before the deadline, the 10th player registration fee will be waived, a savings of $300. After the deadline, The Ultimate Hockey Centre reserves the right to place individually registered players on any teams not having full rosters.

COACH/ASSISTANTS

If you are registering a team, please list the coach and any assistant coach(es)/helper(s) below. Note all coaches and assistants must provide proof of a valid and current criminal records clearance.

If the main contact listed at the top of this form will be the coach for the team, please indicate by checking the box below and only fill in any additional assistants/helpers in the contact fields provided.

Note: Teams must provide an adult (over 19) coach or assistant for all games and on-ice sessions.


Please indicate in the comments section below any allergies or health considerations (ie. asthma)  for any players registered on this form. If you are registering a player to be added to a team already registered, please indicate that and include the name of the team's registrant.

By submitting this form I understand that my child's participation and/or involvement in The Ultimate Hockey Centre's Ultimate 3-On-3 Youth Hockey League and the involvement and participation of any of the players I have registered carries with it the potential for certain risks, some of which may not be reasonably foreseeable.

I further acknowledge that these risks could cause my child or any children registered or others around them, harm, including, but not limited to, bodily injury, damage to property, emotional distress, or death.

By submission of this registration form, I agree on behalf of myself, my child and any/all other players whom I am registering to:

1. Assume all risk of personal injury which may result from participation in the Ultimate Hockey Centre Youth 3-On-3 League. Be advised that all Ultimate Hockey Centre on-ice play requires full equipment. All participants and helpers (coaches, assistants, etc.) are required to wear a CSA approved helmet at all times while on the ice. Children (under19) are required to wear a CSA helmet at all times while on the bench.

2. Release, indemnify, and hold harmless The Ultimate Hockey Centre, as well as all its employees, officials, agents, representatives, successors, etc. from all losses, claims, theft, demands, liabilities, causes of action, or expenses, known or unknown, arising out of participation in orattendance at the Ultimate Hockey Centre's Ultimate 3-On-3 Youth Hockey League.

3. Ensure all participants carry up-to-date private or government health insurance.

4. Be responsible for the cost of any damages caused to the Ultimate Hockey Centre facility or its equipment due to the negligence, abuse or irresponsible behavour of registered players, team helpers/coaches, parents or guests thereof.

5. To grant and release to The Ultimate Hockey Centre the right to use photographs, audio tapes, and/or videotapes in which I and/or my children appear in any materials such as videos, films, recordings, still photographs or articles relating to The Ultimate Hockey Centre, its leagues and services including, but not limited to, brochures, newsletters, annual reports or any other advertising and/or promotional materials whether broadcast on television, radio or any other medium.

6. The Ultimate Hockey Centre reserves the right, without notice to eject and/or suspend temporarily or permanently, at its discretion and without compensation or reimbursement any user of the facility for behaviour that is disrespectful, abusive and/or poses a danger to staff, other players, officials and/or other users/customers/guests and/or poses a risk of damage to the facility and/or its equipment. We expect all players and guests using the Ultimate Hockey Centre to treat the facility with the same respect and consideration they would expect observed at their own homes.

7. The Ultimate Hockey Centre reserves the right to consider, under special circumstances and at its discretion with the approval of the player's parent/guardian adjusting the placement of a player, including moving a player up or down a division when, in the UHC's judgment a player's size and/or skill level is better suited for purposes of safety and/or fair play in a younger or older age division.
